Fermentation Temp? OG?

Little confused how you have a 5.5 gallon batch but 6.76 gallons at the end of the boil (are you leaving 5 quarts behind in the kettle?)

I always leave 3/4 gallon of trub behind in the kettle, and I account for .5 gallon loss due to transfers off the yeast which it gets transferred twice before going into a bottling bucket. My fermentation temp was a true wort temp 65 degrees throughout the fermentation via thermowell, and warming the fermentation to 70 degrees after 5 days of initial fermentation. The original gravity was 1.081, with an ending gravity of 1.012, which brings the beer in at 9.0% ABV.
